- Â Obtain the official application form format provided in the admission notice.
- Â Fill out all required personal details, academic history, and category specifications accurately.
- Â Attach self-attested copies of all necessary supporting documents (10th & 12th mark sheets, caste certificate, experience certificate if applicable).
-  Include a ₹20 Postal Order addressed to the Chief Medical and Health Officer (CMHO) of your target district (SC/ST candidates are exempt).
- Â Send the application envelope via Registered Post, Speed Post, Ordinary Post, or submit it in person directly to the concerned district CMHO office. Do not send applications directly to the Jaipur Directorate.
